Notetaker Evaluation Form

Welcome to ASU's West campus! Because you have registered with us at the Disability Resource Center (DRC), we know you are committed to using any support services that will help you succeed. Therefore, let us match you up with another "seasoned" DRC student who has been here a semester or two and can be a peer mentor to help ease you into this new environment.

To the extent possible, this individual will be an appropriate match based on the characteristics you indicate below and will contact you to introduce him/herself. The two of you can then set up a meeting time and place that is mutually convenient. You can meet in the cafeteria for lunch or coffee or arrange a game of racquetball - whatever both of you prefer. We also invite both of you to attend our Welcome Gathering in September; more information to come.
